Services offered through HART 4000


We provide intake, assessment, referral and financial assistance linked to housing needs. We are NOT an emergency relief service. We aim to provide a comprehensive support plan for clients in partnership with other services. We offer a variety of clinics and projects:

  • QPILCH (Legal)

  • HHOT (Mental Health)

  • ATODS ( Drug and Alcohol)

  • Dental Appointments

  • TICA data base searches

  • Homeless Court Connect Peer Mentor Project

  • HotPAC

  • Free eco-friendly fruit & vegetables

  • Toiletries and toilet paper

  • Second hand clothes

  • Early intervention Boarding House Worker

HART 4000 operates between 9am and 5pm Monday to Friday, with some outreach outside of these hours.  There may be times when people seeking assistance will need to make an appointment or will experience some delay before meeting a HART team member. This is due to the high demand for the service, as well as staff commitments to  outreach and follow up during working hours.

Key Elements of HART 4000

  • Multiple points of access to homeless people via the consortium members

  • A centrally located co-ordination team will ensure assertive approaches to case coordination and will gather visiting services including legal, health, hygiene, storage and housing services

  • The coordination team will maintain links with a broad network of housing providers in the interest of homeless people and their diverse needs (public, community, private)Top

Service Delivery Model


  • Case management model focused on sustainable outcomes and tenancies

  • Housing providers will contribute bed nights and transitional housing to HART 4000

  • Strong partnerships with existing service providers to enhance client outcomes

  • Strong advocacy role in relation to homelessness issues and HART 4000 will be contributing to the development of other State wide initiatives

  • Working closely with the Micah Hub to develop co ordination in relation to the allocation of brokerage dollars and complimentary service delivery processes

  • Understanding Homelessness
